In consideration of the potential gift of heating oil by Tragar Home Services to the Nominee I nominated, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I agree as follows:

Permission Grant: I grant permission to Tragar Home Services, its affiliates, agents, and employees (collectively, the “Sponsors”) the irrevocable and unrestricted right to take, reproduce, distribute, display or otherwise use photographs, video and audio recordings, or images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium.

Release of Claims: I hereby release the Sponsors and their legal representatives for all claims and liability relating to said photographs, recordings, or images. Furthermore, I grant
permission to use my statements, whether written or oral, with or without my name, for the purpose of advertising and publicity without restriction. I waive my right to any compensation.

Age Confirmation: I acknowledge that I am: Over the age of 18 OR the legal guardian of the nominator.

Program Rules: I confirm that I have read and understand the program rules at tragar.com/fuelingkindness.

Nomination Process: During the Nomination Period (November 1, 2024 to April 1, 2025), I may nominate one or more individuals who meet the eligibility criteria by submitting their name, address, and a statement about why they deserve to receive free heating oil, using the nomination form at tragar.com/fuelingkindness.

Nomination Restrictions: I understand that I may not submit more than one nomination for a Nominee, and that I may not nominate myself or a person living in my residence.

Conditions: Tragar is not responsible for any service required after delivery. The recipient releases Tragar of any liability for incurred service determined necessary after delivery.

The recipient releases Tragar of any liability for a tank that fails after delivery. The delivery of heating oil will only be made to a tank in good standing and installed to current building codes. Recipients must be home at time of delivery. This donation does not represent a cash value. The delivery is for a single delivery with a maximum of 150 gallons. This donation is the act of a single delivery and it is presented in full at the time of delivery. Gallons not made and if the delivery is less than the maximum delivery are surrendered upon completion of the delivery and are not transferable. Recipients must allow Tragar to inspect the tank prior to receiving donation of your home heating oil.

Eligibility Criteria: I confirm that my Nominee meets all eligibility criteria as outlined in the program rules, including being a permanent legal resident of Long Island, New York, owning and residing in a single-family home that uses an oil tank for heating, and demonstrating financial need and positive community impact.

Ineligibility for Other Assistance: Applicants must not be eligible for other heating assistance programs, such as the Home Energy Assistance Program (HEAP), or must have exhausted those benefits.

Selection Process: I understand that Tragar Home Services will review each nomination to ensure eligibility and select Recipients based on demonstrated need, community impact,
financial hardship, and other criteria as outlined in the program rules.

Additional Documents: If my Nominee is selected as a Recipient, I agree to sign any additional documents required, including a release of liability and a publicity release.

Program Changes: I acknowledge that the program is subject to change upon notice at tragar.com/fuelingkindness.
